The inspector’s account

On October 1, 2025, Licensing Program Analyst (LPA) Jenifer Tirre conducted an unannounced required visit using the CARE Inspection Tool. LPA was greeted by staff and granted entry after stating the purpose of the visit. Program Manager Shirley Meade and Director of Programs Lisa Crowley was available to assist with the facility inspection. Executive Director Janel Andrews arrived later on to assist with inspection.

The facility is licensed for census of seventy five (75) with thirty five (35) non-ambulatory clients. Facility is an Adult Day Program serving Developmentally Disabled Adults.

At around 8:30AM, LPA conducted a tour of the physical plant accompanied by Program Manager Shirley Meade and Director of Programs Lisa Crowley, and the following was observed: there were no bodies of water on the premises. Day Program is split into two programs Encore and Cava. Each Program has their own roster and workspaces for clients. Physical Plant was inspected, facility is a single story industrial building with several spaces. Day Program has two large common areas, art rooms, meeting room, small classroom for individual therapy, corner store for clients to shop, behavioral management room, office spaces, staff break room, and storage area.

LPA observed storage lockers for the client's personal belongings. Facility has Refrigerator and snack area for clients to store lunches and food. Facility has operational microwaves to warm up food. Facility has five bathrooms. Bathrooms were operational with water temperature measured between 112.0 to 113.7 degrees F. A comfortable temperature of 73 degrees F. was maintained in the facility.
