Residents are being harassed while in care

Licensing Program Analysts (LPAs) Kimberly Lyman and Claudia Gutierrrez made an unannounced visit to the facility to deliver findings on the above allegation. LPAs were greeted and granted entry into the facility by Administrator Patty Osuna and explained the reason for the visit. Care Coordinator Alexis Jones was present as well.

During the complaint investigation, LPA toured the facility as well as interviewed staff and residents. Regarding the allegation that residents are being harassed while in care, the investigation revealed the following: Five out of five staff and five out of five residents deny witnessing any residents being harassed while in care. All five residents interviewed state the staff is respectful and kind. LPA is unable to ascertain any additional information. Therefore the allegation is deemed UNFOUNDED, meaning the allegation was false, could not have happened and/or is without a reasonable basis. Exit interview conducted and a copy of this report was left at the facility.
