Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced case management visit to amend Complaint Control Number 22-AS-20250107143156. Upon arrival, LPA Haddadin met with staff member Sarsi De Venecia, who granted entry to the facility. LPA Haddadin explained the purpose of the visit. An exit interview was conducted with Sarsi De Venecia, and a copy of this report and the amended report were provided at the conclusion of the visit.
