Facility Evaluation Report

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced Case Management visit to the facility to deliver an amended complaint report. Upon arrival, LPA Haddadin was greeted by Licensee, Divina Alipio, who granted access to the facility. The purpose of the visit was explained to the licensee.

During the visit, LPA conducted interviews with two staff members regarding the same complaint, Control No. 22-AS-20250107143156 , to verify consistency and ensure accuracy of the information contained in the amended report. LPA also reviewed the amended document with the licensee and explained that the amendment did not change the original findings of the investigation. The licensee acknowledged understanding of the amendment and the contents of the report.

An exit interview was conducted, and a copy of this Case Management report along with the amended complaint report was provided to the licensee .
