Facility Evaluation Report

Licensing Program Analyst (LPA) Alvaro Ramirez, Jr made an unannounced case management visit in conjunction with Unusual Incident/Injury Report (UIIR) dated June 9, 2025. LPA was greeted and granted entry by Lead caregiver Mark De Los Reyes. LPA explained the reason for the visit. Administrator (AD) Luzviminda Brimuela arrived shortly after.

Per UIIR on June 3, 2025, Client 1 (C1) left the facility and did not return. Per UIIR, on June 3, 2025, at 4:00 pm a missing person report was filed with the Garden Grove Police Department. Per UIIR, C1's mother and the Orange County Public Guardian were notified.

During today's visit LPA reviewed the Physician Report (LIC602) dated February 26, 2025, for C1. Per Physician Report C1 is able to leave the facility unassisted. Per AD on June 14, 2025, C1's mother picked up C1's personal belongings.

A Health and Safety inspection was conducted, and LPA Ramirez observed no Health and Safety concerns during today's visit.

Based on observations and records reviewed during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with facility AD and a copy of this report was provided at the time of exit.
