Facility Evaluation Report
On April 15, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by Administrator Okkyung Coreas after explaining the purpose for the visit.
The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents, and has a hospice waiver for six. The facility is a single story home with four resident bedrooms, three bathrooms, a living room, a dining room, a kitchen, a staff office, and an attached two car garage. LPA accompanied with AD, conducted a walk-through of the interior and exterior portions of the facility. LPA observed that the facility is vacant and that there are no resident's currently in care. The AD advised LPA that a Change of Ownership (CHOW) application has already been submitted for the facility. LPA advised the AD to notify the department if they admit a resident in the future to ensure the facility remains in compliance.
Based on the observations made during today's visit, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with Administrator Okkyung Coreas and a copy of this report was provided.
