Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert conducted an unannounced visit for an Annual Required Evaluation. LPA was greeted and granted entry by Staff #2 (S2) at 8am. LPA met with Administrator (AD) Liz Halahan and explained the purpose of the visit.
The facility is a level three single-story, seven bedroom, three bathroom residence with an additional playroom and staff break area. The facility has a fire clearance for six ambulatory clients. The facility has a census of six clients in care. At time of entry, four clients had already left for work or Day Program and two clients were awaiting transport.
During today's visit, LPA toured the facility and inspected the smoke and carbon monoxide detectors. Both smoke and carbon monoxide detectors were operational. The fire extinguisher was charged and inspected on October 1, 2025. The facility's last fire drill was conducted on March 19, 2026 and a disaster drill will be scheduled by the end of the month The facility temperature was at seventy five degrees. LPA observed night lights in common hallways and measured hot water temperatures in two of three bathrooms. The water temperature ranged between 107.4 to 119.3 degrees Fahrenheit. LPA did not observe any detectable odors and client bedrooms were clean. While touring bedrooms, LPA observed the family room from the sketch is currently a playroom, a staff break room and one client room. Licensee has been working with the City of Fullerton regarding permits for these rooms after the Department's visit in 2025. A deficiency will be cited. A $250 Civil Penalty was assessed for a Repeat/ Failure to Correct Violation.
