Facility Evaluation Report

This unannounced Case Management – Other inspection is being conducted by Licensing Program Analyst (LPA) Sean Haddad for the purpose of conducting additional investigation for Complaint Control Nos. 22-AS-20250829102755 and 22-AS-20250919161621 . LPA met with Administrator (AD) Peggy Ulland and explained the reason for today’s inspection.

During the inspection, LPA interviewed AD, staff, and obtained and reviewed resident records.

Facility representative was advised that at this time further investigation is required. An exit interview was conducted and a copy of this report was discussed with and provided to facility representative.
