Facility Evaluation Report

Licensing Program Analyst (LPA) Fred Arias made an unannounced visit to the facility to conduct a plan of correction visit for deficiencies issued on November 25,2025. LPA was greeted and granted entry by staff. LPA met with staff and discussed the purpose of the visit.

LPA toured the facility and observed a carbon monoxide detector installed and operating in the living room, thus clearing deficiency HSC 1503.2.

LPA observed physical plant corrections were made including removal of wall AC and replacing window screen, thus clearing deficiency 80087(a).

LPA observed a new patio set with umbrella and chairs for clients to use in the backyard, thus clearing deficiency 85087.2(b).

All citations issued on November 25, 2025 have now been cleared.

An exit interview was conducted and a copy of this report was discussed and provided to the facility representative.
