On this day Licensing Program Analyst (LPA) Fred Arias made an unannounced visit to conduct a required annual visit. LPA rang the doorbell and knocked on the door but it appeared no one was present. LPA viewed the kitchen area through the window and did not observe any staff or clients. There was no vehicle present in the driveway. LPA called phone facility phone numbers on file and left a voicemail for Administrator/Licensee. LPA to return at a different time to attempt to complete the visit.
