Facility Evaluation Report

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct a case management visit. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Bob Fiorentino and discussed the purpose of the visit.

LPA gathered documentation for Resident #1 (R1) that was requested by the Department on January 15, 2026. AD informed LPA that they sent the requested documents and provided LPA with email proof that they were sent on March 6, 2026, and provided copies to LPA.

LPA discussed with AD that documents are to be given upon request.

An exit interview was conducted and a copy of this report along with a technical violation were given at the time of the visit.
