Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to conduct an annual inspection when Licensee Kenneth Hunter informed LPA that they would like to close the facility. LPA was greeted and granted entry by Licensee Kenneth Hunter to conduct a case management visit for the purpose to verify a facility closure.
The Department was informed on July 8, 2025, that the facility had officially closed and had no residents in care. The licensee is no longer operating as a licensed facility. LPA was informed that the last resident was moved out on July 1, 2025. The reason for today’s inspection is to confirm the closure of the licensed facility.
LPA toured the physical plant of the one story facility and observed no residents in care. LPA observed the home to be empty and found no evidence of the home operating as a licensed facility. Based on the observations, the facility is no longer operating as a licensed facility and is closed.
This report was reviewed with Administrator Kenneth Hunter and a copy of this report was left at the facility.
