Facility Evaluation Report

Licensing Program Analyst (LPA) Jerome Haley conducted an unannounced case management visit to issue a citations for a deficiency that was discovered during the course of a complaint investigation.

While investigating complaint control # 22-AS-20260126130316, it was discover an uncleared and associated individual was living in the facility.

It is not clear exactly how long the individual was living in the facility; however it was confirmed the individual was living at the facility.

A couple years ago, the individual was removed from the facility due to a personal matter and is not allowed back. At that time the individual moved to another state.

As a result of today’s Case Management visit, a deficiency will be cited.

An exit interview was conducted and a copy of this report and appeal rights were provided.
