Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced Plan of Correction (POC) visit to the facility on January 31, 2025. LPA met with Isangani "June" De Los Trinos at 11:00am. LPA explained the purpose of the visit was to amend the LIC 809-D from the annual visit on January 6, 2025 and to clear the deficiencies from that visit.
June toured the LPA through the exterior of the facility and demonstrated that the side gate was aligned and self-latches. The cracked restroom window was replaced and all screens were functional. Licensee Augustine Au had also emailed photos of the cleared deficiencies by the POC due date.
The Plan of Correction was cleared. A copy of the amended LIC 809-D, the Cleared POC letter and this report was left with Isangani "June" De Los Trinos upon exit.
