Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted entry at 8am by Staff #1 (S1). LPA met with Administrator (AD), Thien Luu, and explained the purpose of the visit.
The facility is a four bedroom, two bathroom, level 4c single story home with an attached garage and an approved fire clearance for four ambulatory clients. The facility currently has a census of four clients in care. At time of entry three of four clients were awaiting transportation to Adult Day Programs.
During the visit, LPA toured the facility and observed the kitchen was clean and appliances were in working order. A weekly menu was posted on the refrigerator and two-day perishable items were observed. Seven-day non-perishables were observed in the pantry and sharps, knives and chemicals were secured under the kitchen sink. Fire and smoke alarms were tested and were operational There is a camera in the common area that is visual only and a cycle machine for clients to use. The facility conducted its last fire drill on September 26, 2025. The fire extinguisher was charged and inspected on October 22, 2025.
Medications were reviewed and were being given as prescribed. A First Aid Kit was observed with all the required items. Facility also has a First Aid Manual.
LPA toured client bedrooms. Three of four client bedrooms have the required furnishings and linens. One of the client bedrooms has two mattresses. AD has physicians' letters on file for this client with our office and Regional Center dated February 15, 2024 due to client behaviors. Two of two bathroom temperatures ranged between 110.6 to 115.5 degrees Fahrenheit.
