Facility Evaluation Report
Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced Required/Annual Inspection visit. Upon arrival, LPA was greeted by Staff (S1) Adonis Sahagun, who granted entry into the facility. LPA explained the purpose of the visit.
During the inspection, LPA and S1 toured the interior and exterior of the facility, including common areas, resident bedrooms, kitchen, garage, and backyard. The facility is a single-story home consisting of six bedrooms, two bathrooms, a kitchen, dining room, great room, and an attached two-car garage.
Smoke and carbon monoxide detectors were tested in common areas and resident bedrooms and were found to be operational. Resident bedrooms contained the required furniture, linens, and adequate storage space for each resident. A fire extinguisher was observed mounted in the kitchen area with the last inspection date of May 5, 2025. A review of the fire drill log showed the last fire drill was conducted on June 5, 2025.
In the kitchen area, sharps and knives were observed to be locked and inaccessible to residents. A two-day supply of perishable food and a seven-day supply of non-perishable food and water were present. All kitchen appliances were operational at the time of the visit.
Restroom toilets and water faucets were functional. Grab bars were securely installed, and showers were free of mold or mildew. Water temperatures were measured between 115.9 and 116.7 degrees Fahrenheit, which is within regulatory limits.
LPA and S1 also toured the backyard, where a shaded seating area was available for resident use. Both side emergency exits were clear of any obstructions.
