Facility Evaluation Report

On today's date Licensing Program Analyst (LPA) William Vanegas made an unannounced visit for the purposes of conducting an annual inspection. Upon arrival LPA met with Administrator (AD) Marjan Arastoo. LPA explained the purpose of the visit and AD was able to assist LPA with the annual inspection. LPA began a tour of the facility and observed the following. AD Marjan Arastoo has a valid Administrator Certificate valid from May 09, 2025 through May 08, 2027.

The facility is a two storied building equipped with 40 bedrooms, 38 shared bathrooms (one in each room). And four shared bathrooms located in the hallways of the facility. The facility is equipped with a laundry area, a kitchen area, and a common area to participate in activities such as watching television, or playing board games. The outside of the facility has an atrium area that is shaded and is used as a smoking section for clients in care. LPA observed a fire place located in the dining area with a screen secured by screws and making it inaccessible to clients in care.

LPA observed the dining area and kitchen area to be clean and free of any mildew and debris. LPA observed kitchen are to have a large refrigerator, a stove, and a microwave. LPA observed kitchen area to have a two day supply of perishable food for the facility, and a seven day supply of non- perishable food, and a sufficient amount of emergency water on hand for the facility. LPA observed facility bedrooms to be clean and free of any hazards, and debris, LPA observed bedrooms to have all required furnishings including a bed, clean linens in good repair; meaning no strains or tears, a lamp, a chair, and enough storage space to store personal belongings.
