The inspector’s account

On this day Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. made an unannounced Plan of Correction (POC) visit in conjunction with complaint control #22-AS-20240603102049 and citation issued on July 2, 2025. LPA was greeted and granted entry into the facility by Administrator (AD) Marjan Arastoo. LPA explained the reason for the visit.

As of August 5, 2025, Deficiency cited under Title 22 Regulation 85088(f)(1)(A)(B)(C) pertaining to Fixtures, Furniture, Equipment and Supplies has been CLEARED.

During today's visit LPA observed that the facility has a working signal system.

LPA Ramirez conducted an exit interview with AD Arastoo and a copy of this report and Letter of Cleared Deficiency has been provided to the facility.
