Complaint Investigation Report
It was alleged that staff do not ensure that the facility remains free of odors. Out of the 10 interviews conducted, 7 interviews did not corroborate with the allegation by stating "there is always a staff cleaning". During the tour of the facility, LPA De Perio observed that the facility was free of odors, and observed a staff member cleaning C1's room. LPA De Perio also observed that facility was equipped with cleaning supplies and disinfectants.
It was alleged that staff do not ensure a comfortable living environment for clients. Out of the 10 interviews conducted, 6 interviews did not corroborate with the allegation by stating that the facility is "good", and 1 interview specifically stated "We have comfy beds here, and good food. I like living here". LPA De Perio observed client bedrooms to be equipped with furniture in good repair and that facility is free of any hazards.
Based on the information gathered during the investigation and review of documents obtained, LPA is unable to ascertain if the allegations indicated occurred as reported. Although the allegations may have happened or is valid, there is not a preponderance of the evidence to prove or refute the alleged violations occurred; therefore, these allegations are deemed UNSUBSTANTIATED.
An exit interview was conducted with AD Arastoo and a copy of this report was provided and explained.
