Facility failed to provide a safe environment for clients

Licensing Program Analyst (LPA) Celine De Perio conducted an unannounced visit to the facility to deliver the findings. LPA De Perio explained the purpose of today's visit, was greeted by facility administrator (AD) Marjan Arastoo.

It was alleged that facility failed to provide a safe environment for clients. LPA De Perio conducted a total of 5 interviews with clients, of which all 5 clients did not corroborate with the allegation by stating that the facility is "good", "safe" "fine" and denied safety concerns. On 07/27/23, LPA De Perio and Licensing Program Manager (LPM) Sheila Santos conducted a tour of the physical plant of the facility, including randomized client rooms and observed no safety hazards and concerns.

Based on LPA’s interviews which were conducted, review of documents obtained, and observations, LPA is unable to ascertain if the allegation occurred as reported. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove or refute the alleged violation occurred; therefore, this allegation is deemed UNSUBSTANTIATED.

An exit interview was conducted with Arastoo. A copy of this report was provided and explained.
