The inspector’s account

Licensing Program Analyst Samer Haddadin conducted an unannounced annual inspection at the facility. Upon arrival, the analyst met with Administrator Melissa Punzalan to discuss the scope and purpose of the visit. The facility is a single-story residence consisting of six bedrooms and three bathrooms. A detached garage is utilized for the storage of emergency food and water supplies. At the time of the inspection, four residents were under care, all of whom were attending a day program. The interior and exterior of the premises were thoroughly toured. All smoke detectors and carbon monoxide alarms in common areas and resident bedrooms were tested and found to be fully operational. Each resident bedroom was equipped with the necessary furniture, linens, and storage space to ensure comfort. Fire extinguishers located in the kitchen and garage were inspected and were last serviced on May 3, 2025. A review of the emergency drill logs confirmed that the most recent drill was successfully conducted on January 8, 2026. Safety measures were verified, including the storage of sharps and knives, which were found locked in the kitchen and inaccessible to residents. The bathrooms were maintained in good condition; toilets and faucets were operational, grab bars were secure, and showers were free of mold or mildew. Water temperatures were measured and recorded between 114.6 degrees Fahrenheit and 113.9 degrees Fahrenheit. The backyard featured a shaded seating area for resident use, and all emergency exit paths were confirmed to be free of obstructions. A review of two resident files and two staff files indicated that all required documentation was present and up to date. Based on the observations made during this inspection, no deficiencies were cited under Title 22, Division 6 of the California Code of Regulations. An exit interview was conducted, and a copy of this report was provided to the Administrator at the conclusion of the visit.
