The inspector’s account

Licensing Program Analyst (LPA) Sam Haddadin conducted an unannounced visit to the facility for the purpose of completing a required annual inspection. Upon arrival, LPA met with Director of Operations, Administration (DOA), Eric Cid-Lopez. During the inspection, LPA and DOA toured the interior and exterior areas of the facility and observed the following: The facility is a one-story commercial building consisting of a lobby, conference room, eight clubhouses (activity rooms), three storage rooms, three staff offices, a staff lounge, two client lunchrooms, eight bathrooms, and an outdoor patio area. The outdoor patio is shaded and contains a designated seating area for clients. LPA observed 55 clients present at the time of the visit. Clients were engaged in classroom-style discussions and receiving assistance from staff. Bathrooms were observed to be clean, free of debris and mildew, with fully operational faucets and toilets. Hot water temperature was measured at 108.8 degrees Fahrenheit. The facility does not provide food service or medication management. Smoke detectors, carbon monoxide detectors, and the facility’s security system were tested by a contracted company, Security Signal Devices, on January 22, 2025. Fire extinguishers were mounted throughout the facility, fully charged, and last serviced on May 30, 2025, as indicated on the service tags. All sharps, toxic chemicals, cleaning solutions, laundry products, and disinfectants were observed to be securely stored and inaccessible to clients. Based on the observations made during today’s inspection, no deficiencies were cited in accordance with Title 22, Division 6 of the California Code of Regulations. An exit interview was conducted, and a copy of this report was provided to the facility.
