Facility Evaluation Report

On April 24, 2026, Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit for the purpose of conducting the Required 1 Year visit. LPA was greeted and granted entry by Administrator Chin-Wen Cheng.

During the walk-through, LPA observed there were no residents in care as the rooms were occupied by the Administrator's family. Administrator Cheng stated that she is currently not operating due to personal reasons. Administrator Cheng stated that she will notify the Department with a decision of whether to keep the license or to close the facility. Administrator Cheng was also advised that the annual licensing fee in the amount of $495.00 is due April 30, 2026 per review of the Licensing Information System (LIS) Facility Transaction History.

Administrator Cheng was advised to contact the Department prior to opening and accepting a new resident.

An exit interview was conducted with Administrator Chin-Wen Cheng, and copy of this report was provided at the end of the visit.
