Facility Evaluation Report
On today's date Licensing Program Analyst (LPA) William Vanegas made an unannounced visit for the purposes of completing an annual inspection. Upon arrival LPA was greeted and granted entry to the facility by facility staff. LPA explained the purpose of the visit and Administrator (AD) Thien Luu was notified via telephone. AD arrived shortly after to assist LPA with the annual inspection. LPA began a tour of the facility and observed the following. AD has a valid Administrator certificate valid from February 14, 2026 through February 13, 2028.
The facility is a one storied home with four bedrooms all used for clients in care, two bathrooms that are utilized by clients in care. LPA observed kitchen area to be clean and free of any mildew and debris. LPA observed kitchen area to have a gas stove, refrigerator, microwave, dishwasher, washer, and dryer all appeared to be in good repair and tested operational. LPA observed there to be a two day supply of perishable food and a seven day supply of non-perishable food a long with a sufficient amount of emergency water on hand.
LPA observed all client rooms to be free of any hazards and debris. LPA observed client rooms to have all required furnishings including a chair, a reading lamp, a bed, clean linens in good repair; meaning no strains or tares, a chest of drawers, and enough storage space to store personal belongings. LPA observed all client bathrooms to be clean and free of any mildew and debris. Water faucets and toilets tested operational. Bathrooms contained all required furnishings such as grab bars, and slip resistant floor matts. Hot water temperature tested between 117.2 and 118.3 degrees Fahrenheit.
