Facility Evaluation Report
Licensing Program Analyst (LPA) William Vanegas made and unannounced inspection for the purposes of an annual inspection. Upon arrival LPA Vanegas was greeted and granted entry by Direct Support Staff Quang Nguyen, and explained the purpose of the inspection. LPA Vanegas began a tour of the home and observed the following.
This is a one storied home with four bedrooms and two bathrooms one of which is a shared bathroom and one of which is a private bathroom. Facility has an attached two garage, and a large backyard. LPA Vanegas observed kitchen area to be clean and free of debris. LPA Vanegas observed a refrigerator, washer, dryer, gas stove, and a dishwasher all to be operational in good repair. LPA Vanegas observed a two day supply of perishable food, and a seven day supply of non perishable food.
LPA Vanegas observed smoke detectors and carbon monoxide detectors to be operational, and in good repair. LPA Vanegas observed all fire extinguishers to be fully charged an up to date. LPA Vanegas observed both bathrooms to be free of debris and mildew. Water faucets and toilets tested operational, and water tested to be between 104.5 and 106.2 Degrees.
LPA Vanegas observed all client bedrooms to be clean and in good repair, no obstructions were noted along the entry way of the bedroom. Bedrooms consisted of all required furnishings such as a lamp, chair, chest drawer, enough storage space for personal belongings, a bed, and clean linens in good repair; meaning no strains or tears.
