Licensing Program Analyst (LPA) Samer Haddadin conducted a case management visit to the facility to amend complaint control number 22-AS-20251104123722. Upon arrival, LPA Haddadin was granted entry and met with Administrator (AD) Louise Igisaiar, and the purpose of the visit was explained. An exit interview was conducted, and a copy of this report was provided to AD Igisaiar.
