Facility Evaluation Report
Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced visit to the facility to complete the required annual inspection. Upon arrival, LPA was greeted and granted entry by Administrator (AD) Louise Igisaiar, and the purpose of the visit was explained. The facility is a single-story consisting of six client bedrooms, two client bathrooms, a living and dining area, kitchen, laundry room, two-car garage. LPA toured the facility and observed the kitchen, dining room, and living room. Smoke detectors and carbon monoxide detectors were observed in the common areas and bedrooms; all were tested and found to be operational. LPA observed that the client bedrooms contained the required furnishings, including beds with linens, as well as adequate closet and drawer space to accommodate each client comfortably. Water temperature measured between 110 and 115.2 Degree F. Common areas were clean, free of hazards, and doorways were unobstructed. The kitchen was inspected, and all appliances were operational. LPA observed that the facility maintained the required two-day supply of perishable food and seven-day supply of nonperishable food. Sharps and knives were secured in a locked kitchen cabinet. Toxic substances were also observed to be locked and inaccessible to clients in a storage closet located in the garage. LPA reviewed three client files and three staff files. All reviewed files contained the required documentation. The fire extinguisher was fully charged and had last been inspected on January 12, 2026. The facility’s last fire drill was conducted on March 3, 2026. LPA also toured the backyard and observed a shaded seating area and adequate space for activities. Based on today’s visit, no deficiencies are being cited at this time under Title 22, Division 6 of the California Code of Regulations. An exit interview was conducted, and a copy of this report was provided to AD.
