Facility Evaluation Report

On today's date, Licensing Program Analyst (LPA) William Vanegas made an unannounced visit for the purposes of conducting an annual inspection. Upon arrival LPA was greeted and granted entry to the faciltiy by facility staff. Administrator (AD) was not present at the time of LPA arrival. AD was notified via telephone and arrived shortly after to assist LPA with the annual inspection. LPA began a tour of the facility and observed the following. AD has a valid administrator certificate valid from July 29, 2026 through July 28,2026.

The facility is a one storied home equipped with six bedrooms all which are all utilized for clients in care. The facility is equipped with two bathrooms that are utilized for clients in care, staff, and visitors. Additionally the facility is equipped with an attached two car garage. LPA observed the kitchen area to be clean and free of any mildew and debris. LPA observed the kitchen area to be equipped with a microwave, gas stove, and refrigerator. LPA observed the facility to have a two day supply of perishable food, a seven day supply of perishable food, and a sufficient amount of emergency water on hand.

LPA observed all resident rooms to be large enough to walk about freely, be free of any hazards and obstructions, and have all required furnishings including the following: A chest of drawers, a reading lamp, a chair, a bed, clean linens in good repair; meaning no strains or tears, and enough storage space to store personal belongings. LPA observed all facility restrooms to have all required furnishings including a shower chair if needed, grab bars, and slip resistant floor matts. LPA observed all water faucets and toilets to be operational, and for hot water temperature to test between 113.2 and 113.5 degrees Fahrenheit.
