The inspector’s account

Licensing Program Analysts (LPAs) Lydia Martinez and Alvaro Ramirez, Jr. conducted an unannounced visit for the purpose of conducting a required annual visit. Facility is an Adult Day Program. LPAs were greeted and granted entry into facility by Administrator (AD) Janet Tounzen. LPAs explained the reason for the visit.

At 8:48 a.m. LPAs, AD Tounzen and Assistant Director Magda Vazquez toured the facility. Facility is a single story establishment that is shared within the same building as a church. The facility has 1 bathroom per every room (total of 3 rooms). LPAs observed all Staff were wearing masks. LPAs observed Clients were involved in various activities throughout the facility including, guided exercise, matching board games and cards. LPAs observed the Clients lunch boxes. Facility provides snacks and will provide lunch in case a Client forgets their lunch. LPAs observed multiple open activity rooms with arts and crafts supplies. LPAs observed all fire extinguishers were fully charged. LPAs observed that the entry of the facility has a check in station. Visitors and Staff are pre-screened for Covid-19 symptoms when they arrive. Hand sanitizer is available throughout the facility. There are pre-cautionary signs for Covid-19 mitigation. Facility is clean and organized. LPAs did not observe any obstacles or hazards inside or outside of the facility.

No deficiencies observed during today's visit. No deficiencies are being cited as a result of this visit. An exit interview was conducted and a copy of the report provided.
