Facility Evaluation Report
On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a case management visit. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit.
The Department received a LIC 624 - Unusual Injury/Injury Report on June 23, 2026 for an incident that occurred on June 20, 2026. It was reported that the foot issue started on 06/20/2026 when staff noticed redness on Client 1's (C1) left toe. On June 20, 2026, C1 was taken first to AFC Urgent Care but it was full It was then C1 was taken to OC Urgent Care, but due to lack of x-ray machine it was suggested C1 be taken to a different urgent care. It was reported that due to insurance C1 could not be seen at multiple urgent cares in one day. It was then reported on June 21, 2026 C1 was taken to Exert Urgent Care due to pain on their left foot. C1's mother met up with staff and C1 at the urgent care. It was reported that x-rays were ordered and C1 was diagnosed with a fracture on left third toe. C1's fracture was treated with buddy tape and a post-operative shoe. C1 had a follow up appointment on June 24, 2026 with primary physician.
LPA Mendivil obtained copies of LIC 602 Physician's Report, Individual Program Plan and post doctor visit documentation.
No deficiencies observed during today's visit. An exit interview was conducted and a copy of this report was provided to facility.
