Facility Evaluation Report
Licensing Program Analyst (LPA) Sam Haddadin arrived at the facility unannounced for the purpose of conducting the required annual inspection. LPA was greeted by Caregiver Enrico Mendoza and later spoke with Administrator Alvin Mendoza via telephone to explain the purpose of the visit. The facility is a one-story home consisting of four client bedrooms, two client bathrooms, a kitchen, dining room, living room, staff room, detached two-car garage, and a backyard. At the time of inspection, the facility had four clients in care in which three were attending a day-program and one was at the facility. During the inspection, LPA observed smoke detectors and carbon monoxide detectors in all required areas, including resident bedrooms and common spaces. All devices were tested and found to be operational. Resident bedrooms were equipped with the required furnishings, including appropriate bedding, linens, and sufficient closet and drawer space. Bathrooms were clean, sanitary, and contained functional fixtures. Water temperatures measured 117.2 degrees Fahrenheit in the first restroom and 116.6 degrees Fahrenheit in the second restroom, both within the regulatory range. Towels and personal hygiene supplies were adequately stocked. The facility was clean, orderly, and free of safety hazards. Passageways were unobstructed. The first aid kit contained all required components. Kitchen appliances were operational, and sufficient food supplies were maintained, including a two-day supply of perishable items and a seven-day supply of non-perishable goods, along with adequate emergency food and water reserves.
