Facility Evaluation Report

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct a case management health and safety check on clients in care to follow up on an incident report that was submitted to the Regional Office on November 4, 2025. LPA was greeted and granted entry by staff. LPA met with Quality Assurance Manager (QA) Merlinda Maglaya and discussed the purpose of the visit.

The incident report stated that on November 3, 2025, Client #1 (C1) had aggressive acts towards clients and staff after arriving home from day program and the CAT team was called for evaluation.

LPA toured the facility and observed that clients were arriving home from day program and watching tv. LPA did not observe any broken windows and the facility was in good repair. Staff informed LPA that when C1 is exhibiting behaviors they isolate C1 and assist other clients into their rooms to ensure their safety. QA informed LPA that they are waiting on a new placement for C1 and that it is taking longer than expected.

QA informed LPA that they will send a timeline of behaviors to LPA by COB November 17, 2025.

Based on today's observations no citations were noted per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with QA and a copy of this report was left at the facility.
