The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by staff. LPA met with Quality Assurance Manager Merlinda Maglaya and discussed the purpose of the visit. The facility currently has six clients in care of which all were seen coming back from day program engaging in activities and napping.

The facility is a one story home with five client bedrooms, one staff bedroom, two bathrooms, kitchen, living room, staff office, dining room, and attached two car garage. The facility appears clean, safe, and sanitary. LPA observed the kitchen to be clean and free of vermin. LPA observed a fire extinguisher in the kitchen to be charged and with a service date of March 10, 2025. LPA observed a two day perishable and seven day nonperishable food supply on hand. LPA observed the knives to be in a locked drawer in the kitchen. LPA observed the toxins and chemicals to be in the locked cabinet under the kitchen sink. LPA observed the centrally stored medication to be in a locked cabinet in the kitchen by the refrigerator. LPA observed the client bedrooms to have all the required components and furnishings. LPA observed the bathrooms to be stocked with paper towels, toilet paper, and nonslip mats. LPA tested the water in the bathrooms to be between 107.7-107.9 degrees Fahrenheit. LPA observed a clean supply of linens located in the hallway. LPA observed the staff bedroom to be locked making it inaccessible to clients in care. LPA observed the staff office to be locked making it inaccessible to clients in care. LPA observed the garage to be locked and stored extra supplies for the facility. LPA observed the backyard to be free of obstructions. LPA observed an in ground pool in the backyard surrounded by a wrought iron fence on two sides that are 5ft tall. The remaining two sides of the pool are surrounded by a block fence that is 5ft on one side and 8ft on the other. LPA observed a shaded seating area for client use in the backyard.
