The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Assistant Administrator (AAD) Angelo Cagatan and discussed the purpose of the visit. AAD contacted Administrator (AD) Omar Tawfik of LPA's arrival. AD gave AAD permission to assist LPA with the inspection via phone call. The facility currently has two clients in care.

The facility is a one story home with four client bedrooms, two bathrooms, living room, dining room, kitchen, and attached two car garage. Facility appears clean, safe, and sanitary. All client bedrooms had the required components and furnishings. LPA observed the restrooms to have soap, paper towels, and nonslip flooring. LPA tested the water to be between 116.2-116.7 degrees Fahrenheit. LPA observed a clean supply of linens in the hall closet. LPA observed the kitchen to be free of vermin. LPA observed a two day perishable and seven day nonperishable food supply on hand. LPA observed the knives to be in the locked cabinet above the kitchen sink, making them inaccessible to clients in care. LPA observed the chemicals to be in the locked cabinet under the kitchen sink, making them inaccessible to clients in care. LPA observed a fire extinguisher charged and with a service date of June 2, 2025. LPA observed the centrally stored medication to be in the locked cabinet located in the dining room. LPA observed the backyard to be free of obstructions. LPA observed a shaded seating area for client use. LPA observed two sheds in the backyard used for the landlords storage. LPA observed the garage to be locked with extra supplies and chemicals.
