Facility Evaluation Report

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced case management visit in regards of confirming a 30-day eviction notice. Upon arrival, LPA was greeted by Executive Director (ED), Troy Byington and was granted entry into the facility. LPA conducted a record review of the resident in question of the eviction and found that the resident’s file did not contain a 30-day eviction notice. LPA also interviewed the resident and confirmed that the resident was not given a 30-day notice.

The facility is planning to move forward, at a later date, with the eviction. LPA advised ED to submit the 30-day eviction letter to Community Care Licensing for approval.

An exit interview was conducted and a copy of this report was provided to ED.
