Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced visit to the facility and was granted entry by Administrative Assistant (AA) Sonya Tatunts. The purpose of this visit was to amend complaint number 22-AS-20251015143305. LPA reviewed and discussed the amendment with facility representatives.
An exit interview was conducted, and a copy of this report, along with the amended complaint report, was provided to Administrative Assistant (AA) Sonya Tatunts .
