Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Operations Manager (OM) Glenn Navarro and discussed the purpose of the visit. The facility currently has six residents in care.
The facility is a one story home with six resident bedrooms, four bathrooms, kitchen, living room, dining room, attached two car garage and a backyard with a pool. The facility appears clean, safe and sanitary. All resident bedrooms have the required components and furnishings. LPA observed no obstructions on any facility exits. LPA observed the restrooms to have paper towels, toilet paper and non-slip mats in the shower. LPA tested the water to be between 108.5 and 111 degrees Fahrenheit. LPA observed a clean supply of linens in the hall closet located by the bathroom closest to the kitchen. LPA observed a two-day perishable and seven-day nonperishable food supply on hand. LPA observed the kitchen to be clean and free of vermin. LPA observed the knives to be locked under the kitchen sink and made inaccessible to residents in care. LPA observed the medication to be in a locked medication cart located in the dining room and in a locked closet located next to the kitchen. LPA observed a fire extinguisher in the dining room charged and with a service date of July 14, 2025. LPA observed the chemicals to be in a locked cabinet in the garage and made inaccessible to residents in care. LPA observed the garage to be used as extra storage and office space. LPA observed the backyard to have a shaded seating area for resident use. LPA observed a pool to be enclosed by a perimeter cinder block wall on two sides and is locked by white fences and the patio deck railings making the pool inaccessible to residents in care. LPA observed the backyard to be free of debris and obstructions. LPA and OM tested the carbon monoxide and smoke detectors and they were found to be operational.
