Facility Evaluation Report
Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit for the purpose of conducting a Required/Annual Inspection. LPA met with Administrator (AD) Evelyn Encarnacion and Assistant Administrator (AAD) Jerel Encarnacion and explained the purpose of the inspection.
During the inspection, LPA, AAD, and AD conducted a tour of the inside and outside of the facility, common areas, client rooms, kitchen, and observed the following:
This is a one-story apartment complex with twenty-three (23) client bedrooms and seven (7) bathrooms. All client bedrooms had the required furnishings. All client bedrooms had the required furnishings. LPA observed all client beds had linens and blankets. The courtyard has a shaded sitting area. LPA observed clients participating in church services on the premises and resting in their respective bedrooms. Bathrooms were observed to be free of debris and mildew and faucets and toilets were operational. Water temperature tested between 111.5 – 149.1 degrees Fahrenheit; a deficiency was cited on today’s date. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Smoke detectors and carbon monoxide detectors tested operational. At least one fire extinguisher was observed to be mounted on the wall in every facility hallway. Fire extinguishers were fully charged with service tags dated July 2, 2025. Facility appliances, including gas stove and laundry washer and dryer were observed to be operable. Toxic chemicals, cleaning solutions, and disinfectants were observed to be inaccessible to clients. Medication was observed to be centrally stored and locked. LPA reviewed five client files and two staff files. LPA interviewed five clients and two staff.
