Facility Evaluation Report

Licensing Program Analyst (LPA) Cassandra Mikkelson and Licensing Program Manager (LPM) Laura Munoz met with Administrator Destiny Garcia via Teams to discuss the outcome of the case based on the incident report that was submitted to the Department on 12/04/2025. Interviews and records revealed that on 12/03/2025, R1 reported that S1 had sexually abused them on several occasions. Once the licensee learned of the abuse, the licensee contacted local law enforcement and the Department. S1 was terminated and asked to leave the facility immediately. Based on interviews with other resident’s, R1 was the only resident who was sexually abused by S1. On 12/30/2025, the RO served the facility with an immediate exclusion notice to S1.

As a result of this visit, a deficiency was cited pursuant to California Code of Regulations, Title 22, Division 6, Chapter 8. An immediate civil penalty in the amount of $500 was assessed per Health and Safety Code 1548 for a violation regarding personal rights. Deficiency is listed on 809-D page.

The licensee was informed during today’s visit that a civil penalty is under review and may be assessed at a future date according to Health and Safety Code §1569.49.
