The inspector’s account

On 06/12/2026, Licensing Program Analyst (LPA) Cassandra Mikkelson and Licensing Program Manager (LPM) Laura Munoz met with Administrator Destiny Garcia via Teams to conduct an informal meeting based on the findings delivered on 06/12/2026.

An incident report was submitted to the Department on 12/04/2025. Interviews and records revealed that on 12/03/2025, R1 reported that S1 had sexually abused them on several occasions. Once the licensee learned of the abuse, the licensee contacted local law enforcement and the Department. S1 was terminated and asked to leave the facility immediately. Based on interviews with other resident’s, R1 was the only resident who was sexually abused by S1. On 12/30/2025, the RO served the facility with an immediate exclusion notice to S1.

Licensee responded appropriately to the incident and informed all necessary agencies about the incident.

CCL will continue to monitor the facility to ensure the standard of compliance.
