Facility Evaluation Report
On July 17, 2026 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a follow-up visit to review staff records.
LPA met with the new Administrator Jose Hernandez Rivera. LPA explained all of the proper documentation needs to be submitted by July 24, 2026 or a follow-up visit is warranted and citation will be issued for not having a qualified Administrator.
LPA reviewed a sample of staff files and provided suggestions. Jose was able to provide some training verification for training that has been conducted since becoming Administrator at the facility.
No citations were issued during today's visit.
Exit interview was conducted and a copy of this report LIC809 was provided to Administrator Jose Hernandez Rivera.
