Facility Evaluation Report
On 09/12/2025, Licensing Program Analyst (LPA) Sarah Hurt arrived to the facility unannounced to conduct a Case Management visit based on a Decision and Order. LPA met with facility Regional Director, Francisco Gutierrez, stated the purpose of the visit and was allowed entry into the facility.
A Decision and Order, Notice of Exclusion was served on Staff 1 (S1) on 08/27/2025. Decision and Order came into effect on 09/08/2025. S1 has been excluded from being present or employed by any facility. LPA spoke with facility Regional Director, who stated S1 is no longer with Soledad SVS.
LPA obtained a copy of the current Personnel Report (LIC 500) and verification of facility's Guardian account showing S1 is not associated to the facility. Regional Director agreed S1 will not be allowed to be employed and/or on any facility premises.
Facility Regional Director has received copy of Decision and Order prior to today. LPA has provided a copy of the Decision & Order, Notice of Exclusion and the Declaration of service on today's visit along with a copy of this report.
Exit interview conducted with Regional Director, Francisco Gutierrez, no deficiencies observed or cited on today's visit.
