Facility Evaluation Report

On 5/17/23 at 11:50 a.m. Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct an annual inspection. LPA was greeted by staff member and Administrator (AD) Diana Castaneda was notified. Tour was conducted with Administrator (AD) Diana Castaneda.

LPA toured the facility inside and out. LPA observed fire extinguishers to be last serviced 8/10/22 and in good standing. LPA checked water temperature in one of the men’s bathrooms which read at 109.7 degrees Fahrenheit.

Drinking water is provided to clients and staff from water dispensers.

Facility provided verification of fire drills completed at the facility which were conducted in a timely manner. LPA observed one emergency exit to not be clear and free from obstruction. Citation issued under Title 22, 82020 Fire Clearance

LPA observed clients interacting with staff members. Sample of staff profiles were reviewed. LPA observed current CPR/First Aid certifications in the staff files. Sample of client profiles were reviewed.

Samples of MARs and Centrally Stored Medication Log was reviewed.

LPA observed uncleared staff member at the facility. Citation issued under Title 22, 82019 Criminal Record Clearance

Citations were issued for the deficiencies.

Sample of staff's current DSP training, Med Tech training, mandated reporting policy will be provided to LPA by end of business day 5/22/23.

Exit interview was conducted an copy of LIC809, LIC809D, LIC421BG, and appeal rights were provided to Administrator.
