The inspector’s account

On June 16, 2026 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a case management visit. LPA was allowed entrance into the facility and met with Administrator Michelle Schoop.

Incident report stated R1 was having suicidal ideations. Due to staff not being able to calm and redirect R1, emergency services was called and R1 was taken to the hospital.

R1's hospital stay was for 2 days and then R1 returned to the facility. R1 has been at the facility since May 2025 and this is the first incident where emergency services needed to be called. Since R1's return home there has been no other issues. R1 currently does not attend day program and is encouraged by staff to attend a day program.

LPA was not able to speak with R1 due to them sleeping.

LPA did not observe any deficiencies and no citations were issued during today's visit.

Exit interview was conducted and a copy of this report LIC809 was provided to Administrator Michelle Schopp.
