03/30/2026 and conducted by Evaluator Brianna Miranda

COMPLAINT CONTROL NUMBER: 24-AS-20260330122158

Staff do not ensure that residents are taking their medications as prescribed.

On April 23, 2026, Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to deliver findings for a complaint of the allegations listed above. LPA met with Administrator Elizabeth Reynaga & Resident Care Director Natalie Levario.

Regarding the allegation: Staff do not ensure that residents are taking their medications as prescribed. LPA conducted various interviews. No interviewee’s stated there was issues with medication.

No deficiencies were found regarding the allegation listed above.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

Exit interview was conducted and a copy of this report LIC9099 was provided to Administrator Elizabeth Reynaga.
