Complaint Investigation Report

The Department has investigated the allegation of: Staff consumes resident's meals without consent. LPA conducted multiple interviews with staff and third parties which stated they have never heard of or seen staff eating off the resident's plate.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

Exit interview was conducted and a copy of this report LIC9099 was provided to Administrator Elizabeth Reynaga.
