The inspector’s account

02/10/2026, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. Facility has an emergency disaster plan as required. Facility has an infection control plan as required. There are currently four clients in care. Upon arrival all four clients were present. LPA was greeted by Licensee/Administrator Robert Zhai.

LPA and Licensee toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food.

Facility is a two story building with the garage located on the lower level. All client rooms, bathrooms and common areas are on the main floor. All rooms were furnished per regulation. Extra hygiene products and linens were available. All exits were free from obstruction. Water temperature in sinks accessible to clients in care were measured and found to be within the range of 105 to 120 degrees F. Fire extinguishers were last inspected August 2025. Smoke/Carbon Monoxide detectors located throughout the facility were tested and operational. Sharps were found to be locked and secured in the kitchen cabinet. Chemicals were found to be inaccessible to clients in care. Medications were found to be centrally stored and properly recorded. Facilities last emergency disaster drill was conducted 10/22/2025.

LPA conducted a review of four client records and P&I monies. All records had the required documentation. LPA conducted review of four staff records/training. Upon a review of staff records, LPA found all staff to have required annual and initial training as well as current 1st Aid & CPR certification on file.

Administrator certificate for Qiuming Zhai (7001408735) is current and expires 11/20/2027.
