Facility Evaluation Report
07/14/2025, Licensing Program Analyst (LPA) Loera arrived unannounced to continue an Annual Required inspection that was initiated on 07/03/2025, and was greeted by Administrator, Elise Elliot. Both clients were attending day program during inspection.
LPA conducted a review of two client records. All records had the required documentation.
LPA conducted review of two staff records/training. Upon a review of records, LPA found all staff to have required training as well as current 1st Aid & CPR certification on file.
LPA conducted spot medication count and found medication to be properly recorded on the Centrally Stored Medication log. LPA reviewed client cash resources.
Updated copies of the following documents were requested for facility file and are to be submitted to CCL by 08/14/2025:
LIC400 - Affidavit Regarding Client/Resident Cash Resources
Exit interview conducted and copy of report was provided to Administrator.
