Facility Evaluation Report
On 06/02/2026 at approximately 09:20AM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct 1-Year Required visit of this licensed Adult Residential Facility (ARF) and was greeted by staff member Marien Benoit. Licensee, Walvelt Hyppolite arrived shortly after. Upon arrival, LPA was informed that there were five (5) clients in care and two (2) staff members on-site. The facility’s Fire Clearance has been approved for capacity of six (6) ambulatory clients. Facility is a two (2) story building with six (6) client bedrooms, one (1) staff office, two (2) bathrooms, and common spaces. During inspection there were only two (2) clients at the facility.
At approximately 10:00AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation.
At approximately 10:30 AM, the LPA and Administrator conducted a tour of the building and grounds, which were observed to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted. The amount of fresh and nonperishable foods is within regulation. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. Toxins are stored in a locked under the kitchen sink. LPA observed sharp items such as knifes used for food preparation were rusted and damaged (Technical Violation Given). Water temperature measured 114.6 degrees F and 112.8 degrees F which is within regulation between 105- and 120-degrees F at faucets accessible to clients. Fire Extinguishers found to be last charged in May 2026 at the time of visit. Carbon Monoxide and smoke detectors were present. There was enough lighting in all common areas, client rooms, and hallways. Medication is centrally stored and secure.
