Facility Evaluation Report
On 9/25/2024 Licensing Program Analyst (LPA) Sarah Hurt arrived unannounced at the facility listed above to conduct a continuance of the Annual Inspection which was originally started on 09/30/2025. LPA introduced self, stated the purpose of the visit, and was granted entry to the facility. LPA met with staff Kathy Mckinney, and Caregiver Tiffany Standley. Facility staff present notified facility Administrator Mistie Rose of Annual continuation inspection.
LPA reviewed a sample of facility staff files, and a sample of resident files. LPA reviewed resident Centrally stored medication records along with medications.
Facility Resident 1 does not have required annually updated assessment.
Resident 2's Appraisal does not document recent frequent falls, and is more than 12 months old.
The following Deficiencies are being cited Per Title 22 Regulations.
